Please complete the following forms and save them to your computer as a PDF and email the PDF to our Registrar [email protected].  Once received, she will be in touch regarding the final steps to register your child.  Please note that the health and medical forms do not need to be filled out by a doctor in order for you to email the forms to the Registrar.
